Every new patient fills out a short intake form: contact details, insurance, a brief history of your hearing and general health, and the medications you take. It takes a few minutes and it makes the appointment itself better, because we start the conversation already knowing the basics.
Call the office and we will take care of the paperwork with you. We can go through it on the phone when you book, put a copy in the post, or simply have it ready at the front desk when you arrive. Whichever is easiest.

What to bring to your first visit
- Your insurance card, and any Medicare or Medicare Advantage details. We verify your hearing benefit before your appointment so the cost conversation happens up front
- A list of your medications, including anything over the counter
- Any previous hearing test results, if you have them
- Someone whose voice you struggle with most, if they can come. Hearing a familiar voice during the evaluation tells us more than a recorded word list does, and two sets of ears retain more of what gets discussed
There is no need to arrive early to fill anything in. Come at your appointment time and we will sort the paperwork together.
A note on privacy
We do not collect health information through this website. Your intake form is completed with us, on paper or over the phone, and your records are handled in line with our HIPAA notice and privacy policy.
