HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Northeast Hearing Solutions, Inc. is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to abide by the terms of this notice.

How we may use and disclose your health information

  • Treatment, to provide, coordinate and manage your hearing care, including sharing information with other health professionals involved in your care (for example, your physician or an ENT specialist).
  • Payment, to bill and collect payment for services, including submitting claims to your insurance plan and verifying coverage.
  • Health care operations, for activities such as quality review, training and business management that support your care.
  • Appointment reminders and care communications, we may contact you about appointments, follow-ups and hearing-related services.
  • As required by law, including public health reporting, health oversight, and other disclosures required or permitted by law.

Other uses and disclosures not described here will be made only with your written authorization, which you may revoke at any time.

Your rights

You have the right to: inspect and receive a copy of your health records; request corrections; request restrictions on certain uses and disclosures; request confidential communications; receive a list of certain disclosures; receive a paper copy of this notice; and be notified of a breach of unsecured PHI.

Complaints and contact

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint. To exercise any right in this notice, contact us or call (570) 373-5000.
