- Treatment, to provide, coordinate and manage your hearing care, including sharing information with other health professionals involved in your care (for example, your physician or an ENT specialist).
- Payment, to bill and collect payment for services, including submitting claims to your insurance plan and verifying coverage.
- Health care operations, for activities such as quality review, training and business management that support your care.
- Appointment reminders and care communications, we may contact you about appointments, follow-ups and hearing-related services.
- As required by law, including public health reporting, health oversight, and other disclosures required or permitted by law.
Other uses and disclosures not described here will be made only with your written authorization, which you may revoke at any time.
Your rights
You have the right to: inspect and receive a copy of your health records; request corrections; request restrictions on certain uses and disclosures; request confidential communications; receive a list of certain disclosures; receive a paper copy of this notice; and be notified of a breach of unsecured PHI.
Complaints and contact
